Fenix Health Medical Group physicians are primary care physicians for the following HMO plans. If you have HMO insurance and would like a Fenix Health Medical Group physician, simply call the phone number listed on your insurance card.

Our physicians also accept many PPO, POS and other types of insurance plans. If you have any questions on PPO plans, please call (909) 865-2332.

UM decision making is based only on appropriateness of care, service and existence of coverage. Fenix Health Medical Group, does not specifically reward practitioners or other individuals for issuing denials of coverage or service care. Financial incentives for UM decisions makers do not encourage decisions that result in underutilization. The Utilization Management (UM) staff is available for callers with questions about approvals, denials, or UM criteria eight hours per day, Monday - Friday. Patients can call 1-866-230-2835.
